Fabregas joined Chelsea from Barcelona in 2014 and had initially been a key player for Jose Mourinho and Antonio Conte, winning the Premier League title under both.
"Cesc Fabregas has, today, completed a permanent transfer to Monaco", Chelsea said on their website, just as Ligue 1 Monaco tweeted a video of Fabregas with a voice-over saying "now it's time".
Fabregas put pen to paper on a three-and-a-half-year deal at the Stade Louis II after passing a medical earlier in the day, as he teams up with former Arsenal teammate Thierry Henry on the Mediterranean coast.
The Spanish midfielder began his career in England at Arsenal after joining them from Barcelona's academy and was named as the club's captain in 2008.
The 31-year-old signed a three-and-a-half-year-deal with the French club.
Fabregas scored 22 goals for the London club in 198 appearances. "It is a quality squad with young players and a young coach", said Fabregas, referring to former France striker Henry.
Fabregas fell out of favour at Stamford Bridge since the arrival of Italian manager Maurizio Sarri in the summer, and was keen to move on to get more first-team opportunities.
Vice-president Vadim Vasilyev added: "We are very pleased to welcome to AS MonacoCesc Fabregas, a world-class player and a great professional, whose arrival demonstrates the ambition of the club. We have a big game against Marseille on Sunday, I'm very excited".
However, they have won just twice in nine league games since Henry's arrival and now languish in the relegation zone, five points from safety before the weekend's matches.
